Jewish Family Services (JFS), a respected, not-for-profit organization is offering an opportunity for a Case Manager to work in their Family Care Program. This full-time position is responsible for providing assessments, case management services, advocacy and ongoing monitoring of services to elderly persons living in their own homes.
Qualified candidates must have a Bachelor’s degree in human services or related human services field and have a minimum of 1 year prior experience working with older adults. Prior experience must be from paid positions. The exceptional candidate will have experience in working with adults who have physical and/or developmental disabilities. Candidates must have a working knowledge of Microsoft Office applications and the Internet, and be able to collaborate and work effectively with clients, agency personnel, volunteers and other public and/or private social service organizations. The ability to pass a criminal background check, a valid driver’s license and access to a vehicle with insurance is also required.
