Jewish Family Services - Care Managers

Jewish Family Services (JFS), a respected, not-for-profit is offering an opportunity for two full time Care Managers to work in their Family Care Program. These full-time positions are responsible for providing assessments, case management services, advocacy and ongoing monitoring of services to elderly persons living in their own homes. Duties include but are not limited to:

- Performing care management services according to the policies and procedures of JFS and the Milwaukee County Department of Family Care, and other funding sources, that include conducting assessments of the client’s psychosocial history, monitoring member outcomes related to physical environment, mental and emotional health, social supports, community integration, employment, financial issues, spirituality, and end of life issues based on the member’s assessment
- Completing care plans and other case recordings in a timely and efficient manner.
- Maintaining a minimum 85% productivity or the equivalent in billable time.
- Participating in triage meetings and collaborate with agency staff and other community professionals.
- Advocating for client(s) and client groups.

Qualified candidates must have a Bachelor’s degree in human services or related human services field and have a minimum of 1 year prior experience working with older adults. Prior experience must be from paid positions. The exceptional candidate will have experience in working with adults who have physical and/or developmental disabilities. Candidates must have a working knowledge of Microsoft Office applications and the ability to perform Internet searches. Familiarity with social work methodology is preferred. Strong organizational and interpersonal skills are required. Ability to collaborate and work effectively with clients, volunteers, agency staff and others is required. Knowledge, understanding and/or willingness to learn Jewish culture and traditions necessary. A valid driver's license AND access to a car with insurance is required. Successful completion of a criminal background check with the Wisconsin Bureau of Justice, Crime Information Bureau is required.
